Best Practices for a Healthy Liver (and Why “Liver Detox” Is the Wrong Idea)
The liver doesn’t need a detox. It’s one of the hardest-working organs in your body, built specifically to filter, process, and neutralise everything from metabolic waste to medications, and it does this continuously, on its own, without help from a herb or a cleanse. What the liver does need is protection from the things that overburden it in the first place — and that’s a much more useful conversation than “detoxing” it.
As a doctor, this is the framing I want to correct before anything else in this post: you can’t “flush toxins” out of a healthy liver any more than you can out of a healthy kidney. What you can do is reduce the daily load your liver has to deal with, and support the diet and lifestyle factors that keep it working well. That’s what this post is actually about.
What the liver actually does
The liver sits at the centre of your metabolism, digestion, and detoxification systems. A few of its key jobs:
- Metabolising nutrients — processing carbohydrates, fats, and proteins from food into forms your body can use
- Detoxification — filtering and neutralising substances like alcohol, medications, and metabolic waste products
- Producing bile — essential for digesting and absorbing fats and fat-soluble vitamins
- Storing nutrients — holding onto vitamins and minerals and releasing them into the bloodstream as needed
Given how central it is, it’s worth understanding what actually strains it — and increasingly, diet is at the top of that list. While chronic alcoholism has traditionally been the leading cause of liver disease, we now see an increasing trend in liver disease that is not linked to overconsumption of alcohol. So, if you are a teetotaller, it does not mean your liver is safe anymore.
Non-alcoholic fatty liver disease: a rapidly growing problem
One liver condition deserves particular attention because of how common — and how preventable — it’s become: non-alcoholic fatty liver disease, now more accurately called MASLD (metabolic dysfunction-associated steatotic liver disease) after a 2023 renaming by major liver associations. The new name reflects what researchers increasingly understand: this isn’t primarily an alcohol problem, it’s a metabolic one, closely tied to diet, insulin resistance, and obesity.
And it’s on a genuinely steep upward trend. Current estimates put global prevalence at roughly a quarter to a third of all adults — a number that’s climbed alongside rising rates of obesity and type 2 diabetes. In India specifically, prevalence in urban populations is estimated even higher, in the 30–40% range. What makes this especially concerning is that a meaningful share of people diagnosed with fatty liver disease have never had a drinking problem at all — this is squarely a disease of modern diet and lifestyle patterns, not alcohol.
Why this matters, and what it can lead to
MASLD often starts silently, with no symptoms at all, and is frequently picked up incidentally on a routine abdominal ultrasound. Left unaddressed, it can progress through stages:
- Simple fatty liver — fat accumulation in liver cells, often with no symptoms
- MASH (metabolic dysfunction-associated steatohepatitis) — when fat accumulation triggers inflammation and liver cell damage. This is also linked with deranged blood sugars and lipids.
- Fibrosis and cirrhosis — progressive scarring of liver tissue, which can become irreversible
- Liver failure or liver cancer — the most serious long-term complications, in advanced, untreated cases
It’s also worth knowing that MASLD isn’t confined to the liver — it’s associated with meaningfully higher rates of cardiovascular disease, which is really a reflection of the same underlying metabolic dysfunction affecting multiple systems at once.
The concerning part for me, both as a doctor and as a parent, is that these patterns increasingly start in childhood. Diet and lifestyle habits formed early lay the groundwork for chronic disease that often doesn’t show symptoms until decades later — which is exactly why the foundational, food-first approach I coach isn’t just about how you feel today. Also, the lifestyle you give your children today, is the one they will follow in the future. So, you are the driver for your child’s health as an adult.
What foods contribute to fatty liver disease
The dietary drivers of MASLD are now well understood, and they overlap heavily with the same patterns that damage the gut microbiome:
- Excess added sugar, especially fructose — high intake of sugary drinks, packaged desserts, and foods with added fructose (particularly high-fructose corn syrup) is strongly linked to fat accumulation in the liver, since excess fructose is processed almost entirely by the liver and readily converted to fat.
- Refined carbohydrates — white bread, white rice, and other refined grains contribute to insulin resistance, a key driver of fatty liver disease.
- Ultra-processed foods generally — these tend to combine refined carbohydrates, added sugar, and unhealthy fats in ways that compound the metabolic burden.
- Excess saturated and trans fats — found in fried foods, packaged baked goods, and many processed snacks.
- Excess overall caloric intake — leading to weight gain and obesity, one of the strongest risk factors for MASLD.
Notice that alcohol isn’t on this list — that’s precisely the point of the renaming. While alcohol can contribute towards MASFLD, it is not the sole driver. This is a diet-and-metabolism-driven disease, and the good news that comes with that is it’s also largely a diet-and-metabolism-reversible one.
How fatty liver disease can be reversed
Unlike more advanced liver conditions, early-stage fatty liver disease is genuinely reversible — and diet and lifestyle changes are the primary tool for reversing it, not medication (there’s currently no approved drug that reverses fatty liver on its own, though a few have recently entered use for more advanced disease). What actually works:
- Gradual, sustainable weight loss — even a modest reduction in body weight (in the range of 5–10%) has been shown to meaningfully reduce liver fat.
- Cutting added sugar and refined carbohydrates, especially sugary drinks — this is consistently one of the highest-impact changes.
- Building meals around whole, fibre-rich foods — vegetables, legumes, whole grains, and fruit support both liver and gut health simultaneously, and fibre plays a direct role in improving insulin sensitivity.
- Choosing healthier fats — favouring olive oil and fatty fish over fried foods and processed vegetable oils.
- Regular physical activity — exercise improves insulin sensitivity and has been shown to reduce liver fat independent of weight loss.
- Limiting or avoiding alcohol — even though MASLD isn’t alcohol-driven, alcohol adds an additional burden on top of an already stressed liver.
This is, in essence, the same foundational approach I coach for gut health generally — because the liver and the gut are closely linked. The gut-liver axis means that gut bacteria, gut permeability, and inflammation all directly influence liver health, so diet changes that support your microbiome tend to support your liver too.
Herbs that support liver health
Alongside the dietary changes above, a few herbs have research behind them for supporting liver health — worth understanding for what they are: reasonable additions to a liver-supportive diet, not a “detox” or a substitute for addressing diet and weight.
Milk Thistle
Contains silymarin, a compound studied for its antioxidant and anti-inflammatory properties, with some research suggesting it may help protect liver cells from oxidative stress. Easily consumed as a tea, it is also available as supplements.
Turmeric
Curcumin, turmeric’s active compound, has well-studied anti-inflammatory and antioxidant effects relevant to liver health. Easy to build into daily cooking — curries, soups, dressings, even a warm turmeric latte before bed.
Ginger
Has anti-inflammatory and antioxidant properties that may support liver function. Works well fresh in cooking or as a tea with lemon.
Dandelion
Traditionally used in Chinese medicine for liver support; dandelion greens can be added to salads, or the root brewed as a tea.
Peppermint
May offer some liver-protective and digestive benefits. Easy to grow at home and simple to add as a tea or garnish.
A word on “liver detox” supplements
This is where I want to circle back to the framing at the top of this post. Over-the-counter “liver detox” supplements are a booming, largely unregulated market, and the marketing claims attached to them consistently outpace the evidence.
A few things worth knowing before you reach for one:
- Regulation is inconsistent. In many countries, herbal supplements aren’t held to the same manufacturing and quality standards as medications, meaning potency and purity can vary widely between products.
- Some are genuinely harmful. Certain herbs used in detox products — kava and comfrey among them — have documented associations with liver damage and toxicity, which is a fairly striking irony for products marketed as liver protective.
- “All natural” isn’t the same as “evidence-based” or “safe.” Many products lean on this kind of language without the research to support their specific claims.
None of this means herbs have no place in liver health — the ones discussed above genuinely do, in food-based amounts, as part of an overall diet. It means being skeptical of concentrated supplement formulations promising to “detox” or “cleanse” your liver in a way your body wasn’t already doing on its own.
The bottom line
Your liver doesn’t need rescuing with a cleanse — it needs a lighter, more consistent load. Given how tightly linked fatty liver disease is to modern diet patterns, the single highest-leverage thing you can do for your liver is largely the same thing that supports your gut: cut back on added sugar, refined carbohydrates, and ultra-processed food, build meals around whole plant foods and fibre, move regularly, and moderate alcohol. Herbs like turmeric and milk thistle are a reasonable, food-first addition to that foundation — not a substitute for it, and not a “detox.”
